I hereby give my permission for the Fairy Godmother Global (hereinafter called FGG to use my videotaped likeness on the FGG website, Facebook page, YouTube channel, Instagram page and in other promotional materials.
I hereby grant permission to the rights of my image, likeness and sound of my voice on audio or videotape without payment or any other consideration. I understand that my image may be edited, copied, exhibited, published or distributed, and I waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording.
I hereby hold harmless and release FGG from all claims, demands and causes of action which I, my heirs, representatives, executors, administrators or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorisation.
I have read the authorisation and release information and give my consent for the use as indicated above.
